    目的 探讨抗幽门螺杆菌（Hp）治疗对Hp阳性寻常型银屑病患者疗效的影响及Hp感染与寻常型银 屑病发病的可能机制。方法 选取2019年12月-2021年9月重庆三峡医药高等专科学校附属医院皮肤科确诊 的50例Hp阳性寻常型银屑病患者，采用随机数字表法分成试验组与对照组，每组25例。试验组予以外用卤 米松乳膏、卡泊三醇软膏联合四联抗Hp治疗，对照组予以外用卤米松乳膏、卡泊三醇软膏治疗，比较两组 治疗前、治疗后1个月PASI评分以及血清IL-17水平。结果 两组治疗后PASI评分低于治疗前，且试验组低 于对照组，差异有统计学意义（P＜0.05）；两组治疗后IL-17水平低于治疗前，且试验组低于对照组，差 异有统计学意义（P＜0.05）。结论 对Hp阳性寻常型银屑病患者进行抗Hp治疗有利于提高患者临床疗效， Hp感染可能通过诱导血清IL-17水平升高而参与银屑病的发生与发展。

    Abstract:

    Objective To investigate the effect of anti-Helicobacter pylori (Hp) treatment on Hp-positive patients with psoriasis vulgaris and the possible mechanism of Hp infection and psoriasis vulgaris. Methods A total of 50 patients with Hp-positive psoriasis vulgaris diagnosed in Department of Dermatology, Affiliated Hospital of Chongqing Three Gorges Medical College from December 2019 to September 2021 were selected and randomly divided into experimental group and control group, with 25 cases in each group. The experimental group was treated with halometasone cream and calcipotriol ointment combined with quadruple anti-Hp therapy, and the control group was treated with halometasone cream and calcipotriol ointment. The PASI scores and serum IL-17 levels of the two groups before and 1 month after treatment were compared. Results The PASI score of the two groups after treatment was lower than that before treatment, and the experimental group was lower than the control group, the difference was statistically significant (P<0.05). The level of IL-17 in the two groups after treatment was lower than that before treatment, and the experimental group was lower than the control group, the difference was statistically significant (P<0.05). Conclusion AntiHp treatment for patients with Hp-positive psoriasis vulgaris is conducive to improving the clinical efficacy of patients, and Hp infection may be involved in the occurrence and development of psoriasis by inducing elevated serum IL-17 levels.
