    目的 比较不同离心条件下制备改良富血小板纤维蛋白(PRF)在治疗重度慢性牙周炎中的作 用。方法 选取2016年9月-2021年9月于我院行翻瓣术的66例重度慢性牙周炎患者为研究对象,根据PRF制 备条件分为低速制备组(n=34)和高速制备组(n=32)。低速制备组采用1500 r/min离心14 min制备改 良PRF(A-PRF),高速制备组采用3000 r/min离心5 min制备改良PRF(L-PRF),比较两组离心后血 清生长因子水平、临床附着丧失(CAL)及牙周探诊深度(PD)。结果 低速制备组离心后血小板源性 生长因子-BB(PDGF-BB)、血管内皮细胞生长因子(VEGF)及转化生长因子-β(TGF-β)水平均 高于高速制备组(P<0.05);两组术后3个月CLA和PD水平均低于术前,且低速制备组低于高速制备组 (P<0.05)。结论 降低相对离心力并延长制备时间得到的A-PRF在生长因子释放性能、促进成纤维细胞 增殖和迁移的作用优于L-PRF,且在促进牙周组织再生和修复中的应用价值较高

    Abstract:

    Objective To compare the effects of modified platelet-rich fibrin (PRF) prepared under different centrifugation conditions in the treatment of severe chronic periodontitis. Methods Sixty-six patients with severe chronic periodontitis who underwent flap surgery in our hospital from September 2016 to September 2021 were selected as subjects. According to the preparation conditions of PRF, they were divided into low-speed preparation group (n=34) and high-speed preparation group (n=32). The modified PRF (A-PRF) was prepared by centrifugation at 1500 r/min for 14 min in the low-speed preparation group, and the modified PRF (L-PRF) was prepared by centrifugation at 3000 r/min for 5 min in the high-speed preparation group. The serum growth factor level, clinical attachment loss (CAL) and periodontal probing depth ( PD) were compared between the two groups after centrifugation. Results The levels of platelet-derived growth factor-BB (PDGF-BB), vascular endothelial growth factor (VEGF) and transforming growth factor-β (TGF-β) in the low-speed preparation group were higher than those in the highspeed preparation group (P<0.05). The levels of CLA and PD in the two groups at 3 months after operation were lower than those before operation, and those in the low-speed preparation group were lower than those in the high-speed preparation group (P<0.05). Conclusion The A-PRF obtained by reducing the relative centrifugal force and prolonging the preparation time is superior to L-PRF in terms of growth factor release performance, fibroblast proliferation and migration, and has high application value in promoting periodontal tissue regeneration and repair.
